Common Questions:
- Do I need a prescription from my physician?
Ohio law requires physical therapy to have a prescription in hand before initiating the evaluation or treatment. We prefer to also have a prescription for occupational therapy or speech-language pathology.
- Who do I need to call to set up an evaluation?
Call our Therapy Service Coordinator for all referrals, scheduling, and general therapy questions.
- Do you file insurance?
We do file primary and secondary insurance, Medicaid and BCMH as a service to our clients. Co-pays are collected at time of service.
- How do I pay for services?
A statement will be issued for all non-covered charges. You can mail or bring in the balance due. We accept cash, check and charge cards.
- What days of the week and hours are you open?
We provide occupational therapy, physical therapy, and speech language pathology Monday through Friday from 7:30 am to 6:00 pm. Our last appointment is scheduled at 5:30 pm.
- How much does an evaluation and treatment cost:
The fee of $150.00 for an evaluation includes 1 hour of clinical assessment, testing, test analysis and a written report. Our individual therapy services are billed in 15 minutes units. The fee is $25.00 per unit. Generally a treatment session is 30 to 45 minutes. We offer group therapy services at a fixed fee of $40.00 per session per discipline. Please call our Therapy Service Coordinator for current rates. Fees subject to change.
- Do you provide transportation?
Transportation is not provided by our facility. We can provide you with
transportation resources in our area.
